Executive Summary
CHOICES Health Services will offer innovative and cost-effective health education and wellness programs to Alachua County residents. Participation will be open to the entire community. The overall goal of the program is to have Alachua County residents participate in educational prevention and wellness programs in order to increase their knowledge and health through improved health behaviors. Alachua County Health Department (ACHD) has been has been selected to administer the program. ACHD will be responsible for providing cost-effective access to prevention and health promotion services; increase the awareness and participation in CHOICES; and improve the health status and quality of life of program participants. Educational programs such as smoking cessation, heart health, exercise, and nutrition will be offered. Programs will be offered in a variety of settings throughout the County. ACHD will collaborate with other community agencies and partners, including the CHOICES Disease Management Program, in order to maximize community resources. The program will also provide worksite wellness programs for selected businesses. Through implementation of the CHOICES Health Education & Wellness Program, CHOICES will be able to reach an increased number of County residents and have a broader impact on the health of our community. This agreement may be renewed by the County for two (2) additional one (1) year terms under the same terms and specifications contained herein.
Background
Alachua County's BOCC recognized the need to provide innovative programs to address specific healthcare needs of County residents; and therefore has implemented the CHOICES Health Services program. The County Ordinance 06-04 and the Indigent Health Care Surtax Sec. 39.10.05: Use of surtax proceeds indicates that proceeds may be used to provide innovative, cost effective disease management/health education programs to Alachua County residents without regard to their income. Sec 39.10.08: The plan for providing a broad range of health care services to indigent and medically poor Alachua County residents may include the following: A health care program that provides for the innovative treatment and management of chronic diseases such as diabetes, hypertension, asthma, and congestive heart failure, that leads to hospitalization that could be avoided by such treatment. The management of chronic diseases will include preventive health care education for qualified Alachua County residents.
Issues
If the agreement is not approved, CHOICES will not be able to provide Health Education & Wellness services to Alachua County residents which has been recommended and approved by the Ordinance.
Fiscal Recommendation
Approve funding for Agreement between Alachua County and the Florida Department of Health Alachua County Health Department for the CHOICES Health Education & Wellness Program. There are sufficient funds for this program.
